I treat on the table first, then teach skills, techniques and tools to manage headaches independently — my favourite is lacrosse ball myofascial release. Muscle-engagement patterns for exercise help the back engage properly, so the headache-contributing muscles work less, and back strengthening plays a bigger role than you'd think — when the back is stronger, it is counterintuitively less tight. That strengthening can be a long-term solution that carries you through many years without having to think about tension headaches.
